Home and community care for older people

Our program helps older residents aged 65 and over – or 50 and over for Aboriginal and Torres Strait Islander residents – stay living at home and connected to their community.

It is funded through the Commonwealth Home Support Programme (CHSP) and managed by My Aged Care.

Services

Individual social support

We can provide support with:

  • accompanied shopping and errands
  • attending social outings, such as visiting a café, park, library or event
  • in-home companionship for shared activities, such as puzzles, meal preparation, short walks, craft activities or conversations with tea.

Home maintenance, repairs and modifications

We can provide support with:

  • home modifications, such as installing grab rails or small ramps (as recommended by an occupational therapist)
  • installing smoke detectors and replacing smoke detector batteries
  • changing light globes and replacing tap washers
  • gardening for safety, such as pruning plants that overhang footpaths and windows
  • spring cleaning, including gutter and window cleaning.

We cannot provide support with:

  • tasks requiring a qualified tradesperson, such as a plumber or electrician
  • painting
  • lawn mowing

We also offer a range of options for group social support.

Referrals and additional help

We can refer you to other community services if needed and our Community Connector Team is ready to help.

Participating in the program

There is an eligibility assessment and referral process to get involved. This may be face-to-face or over the telephone.

Getting a referral

Step 1.Contact My Aged Care

My Aged Care is an Australian Government service that considers your needs and the services you are interested in.

The first step to access government-funded aged care services is to check your eligibility for a face-to-face assessment.

Request an assessment

Step 2.Ask for a referral

If successful, you will be referred for a face-to-face assessment done in person at your own home.

Step 3.Assessment

My Aged Care will then assess your situation and the services available to you. If that assessment finds that you require our services, they will approach us directly with your contact details.

Otherwise, you will be provided with alternative options and support services to contact.

Step 4.Outcome

You can expect to hear from us within 5 business days upon us receiving your referral from My Aged Care.
