Home and community care for younger people

You may be able to access our Home and Community Care Program for Younger People (HACCPYP) if:

  • You are under 65 years of age (or under 50 years of age for Aboriginal and Torres Strait Islander people)
  • You live with a disability, or a physical and/or mental health condition that affects your daily tasks

Eligibility

You may be eligible for Home and Community Care Program for Younger People (HACC PYP) services if you need support to remain living safely and independently at home.

Eligibility is based on the level of difficulty you experience with:

  • Carrying out daily living tasks
  • Remaining at home safely and independently

You may also be eligible if you need support to regain your independence after an accident, illness or hospital stay, or if you or your carer need a break (respite care).

If you are not eligible, or are waiting for services, you can access privately funded services at any time.

Things to know

Community care

We provide support with:

  • Personal care, for example help with showering, getting dressed and grooming
  • Assistance with shopping and errands
  • Meal preparation
  • Domestic assistance
  • Respite and social connection support

Property maintenance

We can provide support for safety purposes with:

  • Home modifications, such as handrails or small ramps (must be recommended by an occupational therapist)
  • Property maintenance for safety purposes, such as installing smoke alarms and replacing smoke alarm batteries
  • Changing light globes
  • Replacing tap washers
  • Gardening for safety, such as pruning plants that overhang footpaths and windows
  • Spring cleaning, window cleaning and gutter cleaning

We cannot provide support with:

  • Tasks that require a qualified tradesperson, such as a plumber or electrician
  • Painting
  • Lawn mowing

Referrals and additional help

We can refer you to other community services if needed and our Community Connector Team is ready to help.

Steps to access HACCPYP

Step 1.Eligibility

Find out if you are eligible for subsidised home care through the HACCPYP services.

Call us to get started

Step 2.Preliminary assessment

Our Community Intake and Assessment Officer will call you to determine if you are eligible for the program within 5 days of your first contact.

Step 3.Face-to-face assessment

After the phone screening, and if you are eligible, the Community Intake and Assessment Officer will organise a time suitable with you to do a face-to-face assessment at your home.

This assessment is scheduled within a timeframe that is suitable based on your priority need.
