Overseas immunisation records form

Use this form to get your family's overseas immunisation records loaded into the Australian Immunisation Register (AIR).

Once submitted, and if required, you will be notified of a vaccine catch-up plan and provided options for immunisation sessions you can attend.

Before you begin

Overseas records must show the:

  • name of the person who has been vaccinated
  • vaccine brands given and what diseases they protect against
  • date of the vaccination
  • name of the vaccine provider
  • batch number of the vaccine given (COVID only).

Records may be submitted in languages other than English. We will contact you if we need additional information or a partial translation of the records.

We will contact you directly after you submit records to collect the $34 fee for this service.
